Who this service is for

This service is available to existing patients of Peak Family Practice who have been seen in person within the past 12 months.

It may be appropriate when:

• Your ongoing chronic treatment has previously been managed by one of our doctors

• Your condition is stable and your treatment plan is already in place.

• A physical examination or new clinical assessment is not currently required.

All requests are reviewed by a doctor in line with safe clinical practice.

What this service includes

Each request is reviewed to confirm that continuation of ongoing treatment remains appropriate.

This includes:

• Review of your medical record and current treatment plan.

• Assessment of whether your ongoing treatment can continue safely.

• Continuation of existing treatment documentation where clinically appropriate.

If a consultation or examination is needed before treatment can continue, we will advise you to book an appointment.

Important notes

To support safe and responsible care:

• This service is available only to existing patients of the practice.

• You must have had an in-person consultation within the past 12 months.

• Some treatments require regular clinical review and may not be suitable for this service.

• New treatment needs or changes to treatment plans require a consultation with a doctor.
