Who this consultation is for

This service is available to patients of Peak Family Practice who have been seen in practice within the past 12 months.

It is suitable when an in-person visit is not practical and a physical examination is not required

This consultation may be appropriate for:

• Discussing acute or ongoing health concerns.

• Reviewing test results and their implications.

• Checking progress and deciding on next steps in care.

• Clinical advice where examination is not required

If an in-person assessment is needed, we will advise you to book a consultation at the practice.

How the consultation takes place

This consultation is usually conducted by telephone. Where clinically appropriate, another method may be arranged.

• This service is available only to existing patients of the practice.

• You must have had an in-practice consultation within the past 12 months.

• New problems requiring examination or more detailed assessment will need an in-person consultation.
