Anti-Wrinkle Injections:
What They Are, How They Work, and What to Expect

by Dr Megan Buckle

Have you noticed lines that linger long after frowning at a screen, squinting in the sun, or simply smiling a lot? Many people explore anti-wrinkle injections (otherwise known as “Botox”) not because they want to look different, but because they want to look more rested, softer, and like themselves on a good day.

At Peak Family Practice we believe that understanding your treatment is just as important as the treatment itself. Here’s what you should know if you’re considering anti-wrinkle injections.

What are anti-wrinkle injections?

Anti-wrinkle injections are a non-surgical medical treatment used to soften fine lines and dynamic wrinkles — the lines that form as a result of repeated facial expressions such as frowning, raising the eyebrows, or smiling.

They use botulinum toxin type A, a purified protein that works by temporarily relaxing targeted facial muscles. By gently reducing muscle movement, the overlying skin is able to smooth out, resulting in a refreshed and natural appearance - not a frozen or “done” look.

When performed correctly, the goal is subtlety: you still look like you, just more relaxed.

How long does it take to work, and how long does it last?

Most patients start to notice an effect within 3–5 days, with full results typically visible by 7–10 days.

The results are temporary, usually lasting 3 to 4 months. Muscle activity gradually returns over time, which is why repeat treatments are needed to maintain results. Many patients find that with consistent, conservative treatment, they can often space sessions further apart over time.

How much will I need?

The number of units required varies from person to person. Factors such as muscle strength, facial anatomy, and your desired outcome all play a role.

As a general guide:

●      Frown lines (between the eyebrows): 16–25 units

●      Forehead lines: 10–20 units

●      Crow’s feet (smile lines): 6–12 units per side

●      Bruxism / TMJ (teeth grinding): ±32 units total

●      Hyperhidrosis (excess underarm sweating): 50 units per armpit

During your consultation, we assess your facial movement in detail and tailor the dose to you — prioritising balance, symmetry, and a natural result.

What products are used?

In South Africa, there are three FDA-approved brands of botulinum toxin type A available:

●      Botox®

●      Dysport®

●      Xeomin®

All work in a very similar way. Differences between them relate mainly to formulation, spread, and how quickly results may appear. If you have a preference, we’re happy to order and use your chosen product - just let us know ahead of time.

A medical, conservative approach

Anti-wrinkle injections are a medical treatment -  and should be approached as such. At Peak, treatments are performed by a doctor, with a focus on:

●      conservative dosing

●      facial harmony

●      natural movement

●      and long-term skin health

We don’t aim to erase every line or change your face. Instead, we support treatments that enhance confidence and help you feel comfortable and well in your own skin.

If you’re curious, unsure, or simply want an honest conversation about whether anti-wrinkle injections are right for you, we’re happy to guide you.
