Who this service is for

The Chronic Care Review service is available to patients who have been seen in person at Peak Family Practice within the past 12 months.

It is suitable when:

• Your current treatment has previously been managed by one of our doctors.

• Your condition is stable and your treatment plan is ongoing.

• No physical examination or new clinical assessment is currently required.

All requests are reviewed by a doctor and handled in accordance with clinical governance guidelines.

What this service includes

Each request is carefully reviewed by one of our doctors to ensure that ongoing treatment remains safe and appropriate.

This includes:

• Review of your medical record and current treatment plan

• Assessment of whether your ongoing treatment remains appropriate

• Continuation of existing treatment documentation where clinically appropriate

If a consultation or examination is required before continuing treatment, we will advise you to book an appointment.

Important notes

To ensure safe and responsible care:

• This service is available only to existing patients of the practice.

• You must have had an in-person consultation within the past 12 months.

• Some treatments require regular clinical review and may not be suitable for this service.

• New treatment needs or changes to treatment plans require a consultation with a doctor.
