Remote follow-up is available to known patients who have been seen in practice at Peak Family Practice within the past 12 months.

They’re suitable when a physical examination is not needed and continuity of care can be safely maintained.

This option supports continuity of care when you’re unable to attend the practice in person and a physical examination is not required.

• Follow-up of acute or chronic conditions.

• Reviewing and discussing test results.

• Reviewing progress and discussing next steps in care.

• Follow-up discussion where visual or physical examination is not required.

If, during the consultation, an in-person assessment becomes necessary, we’ll guide you on the appropriate next steps.
